Hearing loss is not simply about turning up the volume. As people grow older, they may begin to miss certain frequencies, struggle to understand speech in noisy places or feel that conversations sound less clear than before.
Age-related hearing loss, also called presbycusis, usually develops slowly and often affects both ears. It commonly begins with higher-frequency sounds, which help distinguish speech sounds such as S, F and TH. This is why someone may say, “I can hear you, but I can’t understand you.”

What Changes Inside the Ear?

The inner ear contains the cochlea, where thousands of tiny sensory hair cells help convert sound vibrations into signals that travel to the brain.
Over time, these cells can be damaged by aging, repeated noise exposure, certain medications and other health factors. Unlike hair elsewhere on the body, these sensory cells do not naturally regenerate in humans once they are permanently damaged.
Changes also occur beyond the ear itself. Nerve pathways that carry sound information can become less efficient, while the brain may take longer to process speech, especially when several people are talking or background noise is present.
Mayo Clinic audiologist Cynthia Hogan explains that age-related hearing difficulties often involve both the ear and the brain, which is why slower speech, face-to-face communication and reduced background noise can make conversations easier.

Hearing and Balance Are Connected

The inner ear also contains structures involved in balance.
As these systems change with age, hearing difficulties can sometimes occur alongside reduced balance function. Vision and the body’s sense of position also contribute to stability, so several age-related sensory changes occurring together may make falls more likely.
That does not mean hearing loss automatically causes balance problems, but both deserve attention when symptoms appear together.

Common Signs of Hearing Loss

Age-related changes often appear gradually enough that family members notice them first.
Typical signs include:
frequently asking people to repeat themselves;
struggling to follow group conversations;
difficulty understanding speech with background noise;
turning the television louder than others prefer;
feeling that people are mumbling;
finding telephone conversations harder to understand.
Tinnitus — ringing, buzzing, humming or similar sounds without an external source — can also accompany hearing loss.
Because deterioration may be slow, some people compensate without realising how much effort listening now requires.

Hearing Loss Can Be Exhausting

When parts of a conversation are missing, the brain has to work harder to reconstruct them.
This extra listening effort may leave someone unusually tired after meetings, social events or family gatherings. Difficulty following conversations can also lead to frustration or anxiety.
Some people gradually withdraw from activities because restaurants, clubs or group conversations become too difficult to follow. Hearing difficulties have been associated with social isolation and reduced participation in everyday life.
Recognising this connection is important because improving communication may restore much more than sound alone.

When Hearing Changes Need Prompt Attention

Gradual loss is typical with aging. Sudden changes are different.
Seek medical assessment promptly if hearing drops suddenly, particularly in one ear, or if it is accompanied by severe dizziness, bleeding, discharge or major balance problems.
Other causes of hearing loss can include ear infections, eardrum damage, certain medications, repeated loud-noise exposure and some medical conditions.
A hearing test can help determine where the problem originates.

What Happens During a Hearing Test?

An audiologist typically asks about symptoms, family history, noise exposure, medications and everyday listening difficulties.
Testing measures how softly different frequencies can be heard. Bone-conduction testing may also be used to help determine whether the problem involves the outer or middle ear or the inner ear.
Speech testing assesses something equally important: whether words remain clear once they are made loud enough.
The results help determine whether communication strategies, medical treatment or hearing technology may be useful.

When Hearing Aids Can Help

For mild difficulties, simple changes may improve communication.
Turn off unnecessary background noise, face the person speaking and get their attention before starting a conversation. Adjusting television sound settings may also improve clarity without simply increasing the overall volume.
When hearing loss interferes regularly with everyday life, amplification may help.
Modern hearing aids can be adjusted to target the frequencies a person struggles to hear and may use multiple microphones and noise-reduction technology to improve speech understanding.
Hogan notes that hearing aids are not one-size-fits-all; the most useful device is the one selected for a person’s hearing pattern, lifestyle and communication needs.
Some people with mild to moderate hearing loss may also consider regulated over-the-counter options.

Protect the Hearing You Still Have

Age is only one factor affecting hearing.
Keep headphone volume moderate and use hearing protection at concerts, loud workplaces or other noisy environments. If someone nearby can clearly hear what is playing through your headphones, the volume may be unnecessarily high.
Review medication information and discuss potential hearing-related side effects with your healthcare professional when relevant.
Regular hearing checks can also establish a useful baseline, especially when family members begin noticing changes.
Hearing loss may develop gradually, but it should not simply be accepted as something that must be endured. Early testing, better communication habits and appropriate technology can help people remain connected, confident and engaged for longer.
